OKLUSI ARTERI RETINA SENTRAL

Abstract


Abstrak. Oklusi arteri retina sentral merupakan suatu keadaan gawat darurat mata yang dapat menyebabkan kebutaan bila tidak ditangani dengan segera. Kelainan ini sering menyerang orang dewasa yang berusia rata-rata 60 tahun dan berhubungan erat dengan penyakit sistemik seperti hipertensi, kelainan jantung dan pembuluh darah, diabetes mellitus, dan giant cell arteritis. Sumbatan pada arteri retina sentral dapat terjadi akibat emboli, thrombus, plakaterosklerotik, spasme dan inflamasi. Penderita mengeluh penurunan penglihatan yang tiba-tiba, dengan gambaran fundus retina berwarna pucat dan cherry red spot di macula. Penatalaksanaannya disarankan dalam waktu 24 jam sejak timbul gejala dan melibatkan disiplin ilmu lain seperti internis dan kardiologi. Metode terapi meliputi pemijatan bola mata, para sintesis bilik mata depan, pemakaian obat penurun tekanan bola mata, dan terapi hiperbarik. Prognosis penglihatan oklusi arteri retina sentral masih buruk, yang berkaitan dengan lamanya sumbatan. 

Abstract. Central Retinal Artery Occlusion is a true ocular emergencythat can prone total blindness due tolack of immediate proper therapy. This tend to occurs in adult with average ages of 60 years and linked with systemic diseases such as hypertension, cardiovascular diseases, diabetes mellitus, and giant cell arteritis. The occluded retinal artery established by embolism, thrombus, atherosclerotic plague, spasm, and inflammation. Patient with CRAO concerns a sudden decreased vision, with retinal fundus becoming pale with a characteristic “cherry red spot” seen in macula. Immediate therapy suggested to be delivered in first 24 hours after the onset and requires collaboration with internist and cardiologist. The therapeutic methods considered: ocular massage, anterior chamber paracentesis, administration of intraocular pressure reducer drugs, and hyperbaric therapy.  The visual recovery prognosis after CRAO therapy is still unsatisfied, due to the duration of the occlusion.
